Early Intervention Service Coordination

An EI Service Coordinator’s primary responsibilities are to locate, coordinate and monitor services for a caseload of infants and toddlers from birth to age 3 who display developmental delays in one or more of the following areas: Speech and Language, Gross or Fine Motor, Cognition, Self-Help or Social/Emotional Development.

 

The Service Coordinator carries a caseload of approximately 45 cases, and assists the families in accessing an assessment of their child’s functioning, and developing and monitoring the service delivery and an Individual Family Service Plan. Ongoingly, the SC will monitor the services provided, to assure that the child continues to receive the level of care that will best meet their individual needs.
