Why Being a Supports Coordinator is a Fulfilling Career
Here at Expert Community Care Management (ECCM), our mission is simple: to build independence, one empowered consumer at a time. Since our founding in 2006, ECCM has grown from a local initiative in Erie County to a statewide leader in care management and service coordination across Pennsylvania.
At the core of our organization are supports coordinators, who connect children and adults with the resources they need. It is through our dedicated team that every individual receives seamless, conflict-free support.
As the demand for supports coordinators continues to rise throughout Pennsylvania, there has never been a better time to consider this meaningful career path. Join us as we explore why being a supports coordinator is not just a job, but an enriching way to make a difference in your community.
What is a Supports Coordinator?
Support coordination is an important role within the care management system, serving as the bridge between individuals in need and the services that can help them thrive. At ECCM, a supports coordinator is a dedicated professional who guides individuals with intellectual disabilities, autism, or developmental delays through a network of resources.
Responsibilities of Support Coordinator Jobs
Supports coordinators’ primary goal is to empower individuals to lead more meaningful, productive lives. A few support coordinator job duties include:
- Developing and Maintaining Individual Support Plans (ISP): Supports coordinators work closely with individuals and their families to create personalized plans that reflect each person’s unique goals, preferences, and needs.
- Connecting Individuals to Services: They identify the services and supports needed, locate qualified providers, and coordinate access to these resources.
- Advocacy: Supports coordinators advocate for the rights and preferences of the individuals they serve, making their voices heard and respected.
- Care Planning and Monitoring: Those in this role regularly monitor the delivery of services, track progress toward goals, and make adjustments as needed to ensure positive outcomes.
- Securing Health and Safety: Above all, a supports coordinator is committed to the health, safety, and well-being of every individual in their care.
Populations Served as a Disability Support Coordinator
ECCM’s supports coordinators serve individuals with intellectual disabilities, autism, and developmental delays, but their expertise also extends to those with mental health needs.
By following what we call the “Everyday Lives” approach, supports coordinators help people live as independently as possible, often in their own homes and communities. This person-centered philosophy places each individual’s aspirations at the heart of every care plan, guiding supports coordinators to create personalized pathways to independence.
How a Supports Coordinator Makes a Difference in People’s Lives
One of the most rewarding aspects of being a supports coordinator at ECCM is witnessing first-hand the real, positive changes in the lives of the individuals and families we serve. Many of our coordinators have shared stories of the connections they form and the pride they feel when someone reaches a new milestone, whether it’s finding meaningful employment, moving into their apartment, or simply gaining the confidence to try something new.
As one coordinator shared, “There’s nothing quite like seeing someone achieve a goal they once thought was out of reach. Knowing I played a part in that journey is incredibly fulfilling.” Another team member reflected, “Every day, I get to advocate for people’s needs and help them navigate challenges. It’s not always easy, but the moments of progress and joy make it all worthwhile.”
Our supports coordinators aren’t merely guides; they are genuine advocates for the people they serve. By helping individuals access the right services and supports, coordinators empower them to live more independently, all while enjoying a higher quality of life.
