Health Choices Care Management NWPAHealth Choices is a Medicaid Expansion provided by the Pennsylvania Department of Human Services. The purpose of Health Choices is to simplify a complicated process so Pennsylvanians have greater access to the health insurance they need. Erie County Care Management (ECCM) offers child and adolescent care management as well as high-risk adult care management through Health Choices.
