Which Services Are Available in My County?

Care management services are not the same in every county. Some programs are run by the state, while others are run by your county specifically. That means a program your neighbor uses may not run where you live.

How Do I Choose a Supports Coordination Organization?

If you or your child is on a waiver, you get to pick who coordinates your services. You select a supports coordination organization, the agency that helps you plan and arrange your waiver services. Note that this is your choice, not your county's choice.

Your county intake worker will hand you a list of agencies, but they are not allowed to tell you which one to pick. That rule exists to protect you; it means no agency can buy its way onto a list. You can also switch agencies later. If it is not working, you are never stuck.

So the next step is yours! You call the agencies and ask questions, including: 

  • How soon can you start?
  • Who would my supports coordinator be?
  • How often will we talk?
  • What happens if I want a different coordinator?
  • Do you serve my county for the service I need?
