Expert County Care Management (ECCM) is a non-profit organization that is committed to connecting eligible persons and families in Pennsylvania with the appropriate resources and services to improve their quality of life.
Our mission is to ensure everyone has access to the care and support they need, regardless of their circumstances. ECCM offers comprehensive administrative case management for mental health, support coordination for developmental and intellectual disabilities, and early intervention service coordination.

Who Can Get Started with ECCM?
ECCM serves people of all ages, from infants and children to adults, who may benefit from coordinated care.
You may qualify if you or a loved one:
- Have an intellectual or developmental disability
- Have an autism diagnosis or are seeking one
- Need early intervention services for a child under 3
- Require mental health service coordination
- Are at risk of losing housing or need housing supports
- Live in one of the Pennsylvania counties that ECCM serves
Financial Eligibility
- Must meet income guidelines as determined by Medicaid or other funding sources.
Mental Health Eligibility
- Must have a diagnosis based on standardized testing indicating significant impairments in daily functioning (for intellectual disabilities).
- Must have a diagnosis of Autism Spectrum Disorder (ASD) before the age of 22 (for autism support coordination).

The Erie County Emergency Rental Assistance Program (ERAP2) offers rental and utility assistance to those adversely affected by the COVID-19 pandemic. This program helps eligible renters, landlords, and utility providers maintain housing stability, with prioritization given to households experiencing significant financial hardship. Assistance is available for up to 18 months.
