Intellectual Disabilities\Autism Supports Coordination

An IDA Supports Coordinator works with a caseload of approximately 45 individuals with intellectual disabilities or autism. The Supports Coordinator’s primary focus is to locate, coordinate and monitor services for the individuals on their caseload.

 

The Supports Coordinator assists the individual in accessing services, and facilitates an annual Individual Supports Plan, which brings together all of the significant people and services in that individual’s life and results in a comprehensive document that addresses all of the areas of the individual’s life, from demographics to medications, to services being received and informal supports. The Supports Coordinator then monitors the individual and the services being received throughout the year.
