Care Management: Adolescents
 

ECCM offers child and adolescent care management through Health Choices. The Health Choices Child / Adolescent Care Managers are health professionals such as social workers, nurses, or psychologists who will work with you and your provider to plan your behavioral health treatment and recovery. Care managers review individuals' medical needs for mental and behavioral health services including:

  • Residential Treatment Facility (RTF)
  • Community Rehabilitative Residence (CRR) Host Home/Therapeutic Foster Care (TFC)
  • Partial Hospitalization Services (PHP)
  • Acute Partial Hospitalization (APHP)
  • Family-Based Mental Health (FBMH)
  • Psychological Testing
  • Neuropsychological Testing
  • Behavioral Health Rehabilitation Services (BHRS)

What’s the difference between a Residential Treatment Facility (RTF) and a Community Rehabilitative Residence (CRR)/ Host Home/Therapeutic Foster Care (TFC)?

A Residential Treatment Facility (RTF) is a 90-day authorization where children and teenagers live on a temporary basis to get treatment for severe emotional and/or behavioral disorders that cannot be managed in the community. CRR and TFC are for adolescents who can’t remain in the community with only natural resources, but don’t need the structure of an RTF program. With these services, the individual resides with a family extensively trained in working with children experiencing behavioral and emotional difficulty.
