Forensic Case Management - Northwestern PennsylvaniaOften times, individuals with mental illnesses end up in jail for minor offenses which could have been avoided if they were properly treated. If they receive the mental health services they need, they are much less likely to go back again.

 

The mental health forensic case management program at ECCM provides mental health services to individuals with a serious mental illness who are involved in all stages of the criminal justice system. Our ultimate goal is to connect former inmates and individuals who are currently in correctional facilities with proper support and programs to help them accomplish their recovery and wellness goals.

 

Forensic Case Managers

 

Forensic case managers discover eligibility in case management by meeting with individuals in the county jail systems and community re-entry centers. They assess their needs and develop discharge plans for them to work towards. Forensic case managers also help individuals who are released from jail access relevant community resources and coordinate mental health services.
